Requirements for Issuance of Passports

How to download a Passport e-Form? Visit the Passport official portal. Select Passport online services option from the main menu. Passport online services window will be displayed. Select download e-Form from Forms & Affidavits option. Passport e-forms window will be displayed. Select the Fresh/ Reissue of Passport Application option. If you prefer, you can download the new passport application to your computer and open it later. To download the document, follow the instructions provided below. Right click on the link. Select 'Save Target As.' Choose the folder where you want to save the form. Click on 'Save' Download New Passport Application, Form DS-11.

Note: Applications with incomplete requirements WILL NOT be accepted.

CORE REQUIREMENTS:

1.PERSONAL APPEARANCE IS REQUIRED in all cases.2.Pictures will be taken at the Consulate. (Do not bring pictures).3.For passport pictures, applicants are required to wear decent clothing (No plunging necklines, sleeveless shirts, spaghetti strapped tops, see-through tops, sando, tube tops and halters).4.Earrings, necklace, other piercings, glasses and colored contact lenses are not allowed during data capturing.5.If applicant wishes the passport to be mailed back by the Consulate, applicant should BRING a self-addressed stamped envelope such as Certified Priority Mail, Express Mail or a prepaid UPS. Metered stamps will not be accepted .The Consulate General assumes no responsibility for any loss through the mail.

IV.ADDITIONAL REQUIREMENTS FOR FEMALE APPLICANTS WHO WANT TO CHANGE THEIR LAST NAME

1.For applicants who want to use their married name for the first time:a.For applicants married in the Philippines, original and photocopy of Marriage Certificate issued by the Philippine Statistics Authority (PSA, formerly NSO);b.For applicants married outside of the Philippines, original and photocopy of the Report of Marriage (ROM) issued less than a year by the Philippine Consulate where the marriage took place. If ROM is more than 1 year old, bring original and photocopy of PSA Report of Marriage.2.For Females using their married name who want to revert to their maiden name:a.For widowed applicants, original and photocopy of PSA issued Death Certificate of husband and Marriage Certificate;b.For annulled marriages, original and photocopy of PSA Marriage Contract with Annotation on Annulment or Certificate of Registration and certified true copy of Court Order.c.For divorced applicants (allowed only in cases where the non-Filipino spouse acted as the plaintiff; and not allowed when both parties are Filipino citizens at the time of the divorce): Original or certified true copy and one (1) photocopy of Marriage Contract with Annotation issued by the Philippine Statistics Authority (PSA) indicating that the divorce has already been recognized by a Philippine Court.

NEW PASSPORT REQUIREMENTS

A.Accomplished New Regular Passport Application form (Adult) or Regular Passport Application Form (Minor)

I.For Applicants Born in the Philippines

1. 1. Original and photocopy of Birth Certificate issued by the Philippine Statistics Authority (PSA, formerly NSO).2. 2. For married female applicant, original and photocopy of PSA issued Marriage Certificate3. 3. Any of the following valid IDs (original and photocopy)a.Driver’s license b.State IDc.School or Work IDd.Philippine ID (SSS card, Senior ID, School ID, PWD ID or Voter’s ID)

II.For Applicants Born Outside of the Philippines

1.Original and photocopy of Registered Report of Birth issued less than a year by the Philippine Consulate where the birth took place. If ROB is more than 1 year old, bring original and photocopy of PSA Report of Birth.

III.If Applicant is a Dual Citizen

1.Original and photocopy of Birth Certificate issued by the Philippine Statistics Authority (PSA, formerly NSO).Download2. 2. For married female applicant, original and photocopy of PSA issued Marriage Certificate3. 3. Photocopy of Dual Identification Certificate4. 4. Valid Government-issued ID

MINOR APPLICANTS (ADDITIONAL REQUIREMENTS)

1. 1. Accomplished Regular Passport Application Form (Minor)2. Personal Appearance of minor applicant and either parent or guardian. 3. Original and photocopy of Birth Certificate issued by the Philippine Statistics Authority (PSA, formerly NSO).3. 4. In case applicant is not accompanied by the parent/s, applicant must submit a Notarized Special Power of Attorney executed by the parent/s designating the minor’s companion to assist in the application process.The Special Power of Attorney must be authenticated by the Philippine Embassy/Consulate if executed abroad.4. 5. Renewal/ New Passport - $60.00

Lost MRP/ Green/ Brown or older passports - $90.00

RELEASE OF PASSPORTS :6 to 8 weeks from date of application

For first-time applicants, visit the DFA website: dfa.gov.ph, ready your Philippine passport requirements, set a DFA appointment online with the nearest Regional Office to your current address. After that, fill out the online application form and then save it. Print your DFA appointment and bring the aforementioned data to the DFA Regional Office within your region.

What are the Philippine Passport Requirements?

New Applicants of DFA passports MUST take note of these requirements.

  • Personal appearance

  • Confirmed appointment

  • Duly accomplished application form – may be downloaded from DFA Website

  • PSA (Philippines Statistics Authority) Birth Certificate or Certified True Copy (CTC) of BC issued by the Local Civil Registrar (LCR)

  • Valid picture IDs and supporting documents to prove identity.

How to Get a DFA Appointment?

There are actually different ways which I’ve learned where you can get a DFA appointment. Below is a list of them. PASSPORT APPOINTMENTS ARE FREE!

Online Appointment – this is what we recommend for 2018! Access the new DFA online appointment setting system for a DFA passport renewal or new applications. PLEASE DO IT YOURSELF and DON’T PAY A TRAVEL AGENCY TO DO IT FOR YOU! IT’S FREE!

Appointment in the Province. I suggest you prepare the necessary requirements before doing this. After that, you may access the DFA online appointment system and start filling out the necessary details which will be shown in the step-by-step guide in the following subject within this article.

Walk-in appointment. The norm in setting a DFA appointment would mean having to wait long lines since it’s on a first come, first serve basis. You must also pay a fee for the professional skill in setting up a schedule for your DFA passport appointment contributed by the person assigned to do that job in DFA offices.

NO APPOINTMENT NEEDED for the list mentioned below can go to the Passport Courtesy Lane without any prior appointment:

  • Senior citizens (60 years and over) plus one (1) travel companion

  • Minors seven (7) years old and below (with their parents and minor siblings)

  • People with disabilities (PWDs) with PWD ID or visible disability plus one (1) travel companion

  • Single parents (with Solo Parent ID) and their minor children

  • Visibly pregnant women or with medical certificate

  • OFW with sufficient proof of status such as iDOLE Card, valid employment contract, work visa

Important Reminders:

  • All Courtesy Lane applications and facilities including through accommodation or referrals will be treated as expedited applications and should pay the expedited fee of Php 1,200.00.

  • Please be reminded that all applications filed on a Saturday should be charged with expedited fees.

  • Senior citizens have the option to choose regular process priced at PHP 950.00.

  • Courtesy Lane facilities are available in all Consular Offices nationwide.

Back then, I was worried because I thought that if my DFA passport expired, this would mean I would have to get a new one and go back to the basics to accomplish all of the requirements and get it issued again. However, as I read the official website of the DFA, I didn’t need to get a new one at all, I only needed to renew it.

Since I want to speed things up because I want to travel in three (3) months time, I opted for the DFA Online Appointment System, since it was faster, more convenient, and you could just do it at home without having to wait in long lines just to get it done.

If you want to know how this new DFA online appointment system is done, then follow these step-by-step guide to start.

Step 1. Click the link https://www.passport.gov.ph/appointment and read the Terms and Conditions. Mark the checkbox once you’ve read everything and choose whether to apply for an individual appointment or group appointment with either your friends or family.

Step 2. Find the nearest DFA Regional office nearest to your current address - from Region, Country, and Site. Once you choose the site, it will locate the nearest DFA office where you can schedule your appointment.

Step 3. Choose your most convenient schedule to set a DFA appointment for the renewal of your Philippine passport. Set the month, day, and then check for the available time left. Compare your personal daily schedule. After that, mark the time that would suit to your availability and press next.

Step 4. Be careful when inputting your personal info to avoid delays in your appointment and even your processing!

Step 5. Fill in your family information - your father’s name and your mother’s maiden name. Note, the suffixes for your father and mother’s name are to be placed along their first names.

Step 6. Choose your application type if it’s either new or renewal. Fill in your emergency contact person and their contact number. (Your mother, father, sibling, your spouse, or your legal guardian!)

Step 7. Fill in your complete address and even your office address. Note that sections with asterisks are considered very important, so fill them out. Click next after.

Step 8. Mark the check boxes after verifying everything. Confirm to get your appointment code and print your DFA appointment schedule. (Note: the data you input to the page won’t be disclosed to a third party)

Step 9. Check your email for any messages and click it to view your DFA appointment code.

Step 10. This is what the message looks like. Click the highlighted word “here” to confirm and open another page where you can find the next picture,

Step 11. Write down the DFA appointment code and your email address for further verification.

Step 12. After that, download your application form and settle your payment.

Once these are all done, download the file and print your DFA passport appointment. After, print your DFA appointment schedule, your application form, and bring your requirements to your local DFA regional office on the scheduled date of your appointment.

What are the requirements for your Philippines Passport Renewal?

  • Personal appearance

  • Most recent expiring or expired passport

  • Valid picture IDs and supporting documents to prove your identity.

LIST OF ACCEPTABLE IDS (ANY OF THE FOLLOWING WITH ONE (1) PHOTOCOPY):

  • Social Security System (SSS)/ Government Service Insurance System(GSIS) Unified Multi-Purpose Identification (UMID)Card

  • Land Transportation Office (LTO) Driver’s License

  • Professional Regulatory Commission (PRC) ID

  • Overseas Workers Welfare Administration (OWWA) / Integrated Department of Labor and Employment (iDOLE) card

  • Commission on Elections (COMELEC) Voter’s ID or Voter’s Registration Record from COMELEC Head or Regional Office

  • Philippine National Police (PNP) Firearms License

  • Senior Citizen ID

  • Persons with Disabilities (PWD) ID

  • School ID (for students)

  • Current Valid ePassport (For Renewal of ePassport)

  • For applicants based overseas, they may use their host government issued ID’s showing their Philippine citizenship. (Example: Residence Card)
